- Directing the day-to-day operations of the ACT team including overseeing the daily schedule, leading the daily ACT team meetings and treatment planning meetings, and evaluating the status of clients to ensure immediate attention to their changing needs.
- Coordinating the client admission process and treatment, rehabilitation, and support services of the program in coordination with the psychiatrist.
- In conjunction with the business office, report program actual costs to MN Department of Human Services each year to establish a per diem rate.
- Providing oversight of the program’s budget including approving credit card and mileage expenses and ensuring that everyone on the team are good financial stewards.
- Participating in staff recruitment, interviewing, hiring, work assignments, orientation, and performance evaluations of staff.
- Providing staff feedback and coaching on performance via team meetings, monthly supervision, and annual reviews. Providing more frequent individual feedback when needed.
- Ensuring the ACT team is operating to consistently improve and maintain fidelity to the ACT model. Preparing the team to complete the Tool for Measurement of Assertive Community Treatment (TMACT) and promote adherence to State of Minnesota Statutes and Rules.
- Serving as an active member of the team, and participating in all team meetings, duties and activities including taking part in the on-call rotation and being available for consultation when other staff are on-call.
- Strong clinical skills and experience providing treatment to persons with severe and persistent mental illnesses.
- Clear understanding of the characteristics and problems of adults with severe and persistent mental illnesses.
- Strong understanding of evidence-based practices, trauma-informed care, cultural humility, and person-centered language.
- Knowledgeable about the Assertive Community Treatment (ACT) model.
- Ability to motivate and problem solve with staff.
- Ability to collaborate with people from diverse backgrounds and personalities.
- Use effective crisis intervention approaches to reduce the impact of increased symptoms on the client and their caretaker.
- Positive demeanor, empathetic and calm.
- Valid driver’s license
- The team leader must qualify as a Mental Health Professional in Minnesota.
- Master's degree in social work, psychology, psychiatric rehabilitation, or a related clinical field
- Be licensed, or eligible for licensure in their respective field.
- Minimum three years of experience in working with adults with severe mental illness.
- Previous supervisory and program management experience is required.
- Prolonged periods sitting, listening, and observing or working on a computer.
- Must be able to lift up to15 pounds at a time.
- Assist clients in their homes and in the community with various tasks.
- Provide transportation for clients.
Most duties are performed, in an office, homes, or in a community setting. Position requires services in-person and via telehealth.
